Freshness is measured from the last successful fetch, not from the publisher's own release date.A `blocked` outcome (HTTP 403) is not evidence that a source has moved: NHSBSA WAF-blocks automated fetchers.`last_run_outcome` is the outcome of the whole run, not of the HTTP request: a source that answered 200 and then failed to parse reports `drift`, not `ok`.`last_success_at` advances only on a run that produced usable data, so a source that has never been read cleanly is reported stale rather than fresh.`scheduled: false` means the source is in scope but not fetched by the scheduled run (dm+d arrives manually from TRUD). Only scheduled sources can be stale, and only they count toward pipeline_healthy.`in_scope` declares scope, not holdings. `records_held` is the holdings figure: null means not measured, 0 means measured as empty.records_held counts per source, not per nation: applicability is recorded on each event in `nations[]`, so a per-nation split would double-count UK-wide events.records_held counts what reached the ledger, not what the source published: a row the matcher cannot resolve to a dm+d code is queued for review rather than appended, so a source can be read cleanly and still hold fewer records than it listed.
